Rola genu podatności na otyłość (FTO) w rozwoju otyłości — przegląd
Obesity is an epidemic of the 21st century. The magnitude of this problem stems from its increasing prevalence and numerous metabolic complications caused by excessive fat accumulation. The pathogenesis involves both environmental and genetic factors, and FTO (fat massand obesity-associated gene) is one of the most significant among genes predisposing to obesity. The role of FTO polymorphism in the development of obesity has been confirmed in many studies, but the effect varies significantly in different ethnic groups. Moreover, the exact mechanisms of FTO influence are yet to be explained. The association between FTO and lifestyle factors such as diet and physical activity has been extensively studied in recent years. This paper presents current knowledge about the role of FTO gene in the development of obesity and type 2 diabetes in different ethnic groups and the association between FTO polymorphism and lifestyle modifications predisposing to adiposity.Otyłość jest epidemią XXI wieku. Znaczenie tego problemu wynika zarówno z jego narastającej częstości, jak i wielu powikłań zdrowotnych, którym sprzyja nadmiar tkanki tłuszczowej. W patogenezie otyłości istotną rolę odgrywają czynniki środowiskowe i genetyczne, a spośród tych ostatnich jednym z najbardziej znaczących jest gen FTO (gen podatności na otyłość). Wpływ polimorfizmu FTO na rozwój otyłości potwierdzono w wielu pracach, lecz jego bezpośredni efekt zależy od pochodzenia etnicznego osób badanych. Ponadto wciąż niejasny jest dokładny mechanizm działania tego genu. W ostatnich latach intensywnie badano związek polimorfizmu genu FTO ze środowiskowymi czynnikami ryzyka otyłości, takimi jak dieta i wysiłek fizyczny. W niniejszej pracy przedstawiono obecny stan wiedzyo roli genu FTO w rozwoju otyłości i cukrzycy typu 2 w różnych grupach etnicznych oraz jego związek ze stylem życia, który predysponuje do nadmiernego gromadzenia tkanki tłuszczowej
Diagnostic criteria for metabolic obesity in normal weight subjects
Otyłość jest uznanym czynnikiem ryzyka schorzeń sercowo-naczyniowych.
W 1981 roku stwierdzono, że niektóre osoby z prawidłową
masą ciała mają podobne zaburzenia metaboliczne, jakie
występują u ludzi z otyłością brzuszną. Osoby te określono mianem
metabolicznie otyłych z prawidłową masą ciała (MONW).
Zwiększona ilość trzewnej tkanki tłuszczowej u nich wiąże się ze
zmniejszoną wrażliwością tkanek na insulinę, hiperinsulinemią, aterogennym
profilem lipidowym i wyższym ciśnieniem tętniczym. Do
tej pory nie ustalono jednoznacznie definicji i kryteriów rozpoznania
MONW. W dostępnym piśmiennictwie znaleziono 9 sposobów
rozpoznawania MONW stosowanych przez różne grupy badawcze.
Brak jednolitych kryteriów utrudnia diagnostykę, prowadzenie
badań przesiewowych i populacyjnych. Konieczne wydaje się
ustalenie jednolitych zasad rozpoznania tego zaburzenia. W niniejszej
pracy przedstawiono istniejące w piśmiennictwie różne kryteria
diagnostyki MONW.Obesity is recognized as a high risk factor for cardiovascular disease.
The existence of normal - weight individuals who have metabolic
disorders which normally occur among person with visceral
obesity was first proposed in 1981. These individuals were identified
as metabolically obese but normal weight (MONW). The increased
amount of abdominal adipose tissue in MONW is associated
with lower sensibility of tissues to insulin, hyperinsulinemia,
atherogenic lipid profile and higher arterial blood pressure. There
was not created a satisfactory definition of MONW or strictly defined
diagnostic criteria of this disorder. Scientific data on MONW
subjects are scarce since only 9 publications are directly relate to
this topic. There are no uniform diagnostic criteria, it makes difficult
diagnostication, screening and study population. It is essential
to establish uniform criteria for diagnosis of this syndrome. The

Częstość występowania metabolicznej otyłości z prawidłową masą ciała w populacji polskiej
Introduction: In the 1980s, the idea evolved that some individuals with normal weight (Metabolically Obese, Normal-Weight), who probably
have increased abdominal fat, have metabolic disturbances related to obesity. This observation initiated the concept of the metabolically
obese but normal-weight syndrome (MONW). Since then, there have been only a few studies in non-obese subjects. MONW men and
women should be regarded as at high risk for cardiovascular disease.
Material and method: A group of 854 randomly chosen non-obese men and women, 20–40 years of age, was selected from three different
areas of Poland — Szczecin, Krakow and Wroclaw. All subjects were interviewed and underwent physical examination, anthropometric
measurements (waist circumference, hip circumference, BMI and WHR) as well as densitometry (total body DPX, total fat, android/gynoid
deposit). Serum level of fasting glucose and insulin, indices of insulin sensibility (QUICKI) and insulin resistance (HOMA, FIRI), total cholesterol,
triglycerides and HDL-C were measured using commercially available kits. LDL-C level was calculated using Friedewald’s formula.
Results: The total amassed fatty tissue and its android deposit was found to be significantly greater in MONW men and women. MONW
women were found to exhibit increased levels of triglycerides and LDL-C but lower levels of HDL-C. In women with excess abdominal
fat (EAF), fasting glucose and insulin levels, HOMA and FIRI were considerably higher, while QUICKI was lower. Triglyceride and LDL-C
levels were also higher while HDL-C levels were lower. In men with EAF, increased levels of total cholesterol and LDL-C were confirmed.
Conclusions: The occurrence of MONW is contingent upon the diagnosis criterion and increases when the criterion represents the value
of HOMA — 21.76% in women and 31.42% in men. The frequency of MONW occurrence is lower when the criterion for abdominal fat
content limit is used, amounting to 15.78% in women and 7.83% in men. (Endokrynol Pol 2012; 63 (6): 447–455)Wstęp: W latach 80. zaobserwowano, że u nieotyłych osób, które gromadzą nadmierną ilość tkanki tłuszczowej w okolicy brzucha, rozwijają
się powikłania metaboliczne charakterystyczne dla otyłości. Powstała wówczas koncepcja zespołu metabolicznej otyłości z prawidłową
masą ciała — MONW (Metabolically Obese Normal-Weight). W Polsce badania takie nie były dotychczas prowadzone. Identyfikacja ludzi
obciążonych tym zespołem pozwoliłaby na wczesne podjęcie działań prewencyjnych.
Materiał i metody: Zbadano 854 nieotyłych kobiet i mężczyzn, wybranych losowo w 3 regionach kraju – Szczecina, Krakowa, Wrocławia
w wieku 20–40 lat. Przeprowadzono wywiad lekarski, badanie fizykalne, pomiary antropometryczne (obwód talii, bioder, BMI, WHR),
badanie denzytometryczne (total body DPX; total fat; android/gynoid deposit). Ilość i dystrybucję tkanki tłuszczowej oceniano przy użyciu
podwójnej absorpcjometrii energii RTG-DXA. W surowicy krwi oznaczono stężenie glukozy i insuliny na czczo, stężenie cholesterolu
całkowitego, triglicerydów oraz HDL-C i wyliczono stężenie LDL-C oraz wartości wskaźników insulinowrażliwości (QUICKI) i insulinooporności
(HOMA, FIRI).
Wyniki: U kobiet i mężczyzn całkowita ilość tkanki tłuszczowej i jej androidalnego depozytu były istotnie wyższe w grupie z MONW.
Stężenia glukozy i insuliny na czczo oraz wartości HOMA i FIRI były istotnie wyższe, a QIUCKI istotnie niższe u badanych z MONW. U
kobiet z MONW stwierdzano istotnie wyższe stężenia trójglicerydów oraz LDL-C natomiast znacząco niższe stężenia HDL-C. Istotnych
różnic nie było u mężczyzn.
Wnioski: Częstość występowania metabolicznej otyłości z prawidłową masą ciała zależy od kryterium rozpoznania i jest większa, gdy
kryterium to stanowi wielkość HOMA — 21,76% kobiet i 31,42% mężczyzn i mniejsza, gdy posługiwano się kryterium granicznej ilości
tłuszczu brzusznego — częstość rozpoznania MONW wynosi — 15,78% u kobiet i 7,83% u mężczyzn. (Endokrynol Pol 2012; 63 (6): 447–455
Pheochromocytoma in 8-year observation at a single endocrinological center in Wroclaw
Wstęp: Guz chromochłonny występuje rzadko, powoduje zwykle nadciśnienie tętnicze i różnorodne, zmienne objawy kliniczne, co często
utrudnia wczesne rozpoznanie i leczenie. Celem niniejszej pracy było przedstawienie danych klinicznych, efektywności diagnostyki
i leczenia chorych z pheochromocytoma w ośrodku wrocławskim w okresie 8 lat.
Materiał i metody: Wśród 37 chorych z pheochromocytoma leczonych w latach 2000-2007 w Klinice Endokrynologii, Diabetologii i Leczenia
Izotopami Akademii Medycznej we Wrocławiu były 23 kobiety (23-75 lat) oraz 14 mężczyzn (17-74 lat). Zbadano częstość występowania
poszczególnych objawów klinicznych, przydatność zastosowanych metod diagnostycznych i efekty leczenia.
Wyniki: Czas trwania choroby do momentu rozpoznania wynosił od 2 miesięcy do 16 lat. Najczęstsze objawy to: nadciśnienie tętnicze
napadowe lub stałe, tachykardia, bóle głowy, wzmożona potliwość, niepokój. Najczulszą laboratoryjną metodą diagnostyczną był pomiar
stężenia metoksypochodnych katecholamin w moczu dobowym. Tomografia komputerowa u wszystkich chorych wykazała obecność
guza nadnercza, częściej prawego; w 1 przypadku guz był umiejscowiony w ścianie pęcherza moczowego.
Po operacji nadciśnienie tętnicze ustąpiło u 59% pacjentów, obniżyło się u 26,8%, a u 13,9% nie obserwowano poprawy. U 2 chorych guz
był złośliwy, 1 kobieta zmarła po zabiegu. U 8 stwierdzono zespół MEN 2A.
Wnioski: Rozpoznania pheochromocytoma zwykle dokonuje się po długim czasie trwania choroby. Najbardziej typowym objawem jest
nadciśnienie tętnicze napadowe, które występuje jedynie u około 40%, inne objawy są niespecyficzne. Najbardziej przydatne diagnostycznie
jest stwierdzenie podwyższonego wydalania metoksykatecholamin w moczu dobowym. Z badań obrazowych najbardziej efektywna
jest tomografia komputerowa. Chorzy z pheochromocytoma powinni być rutynowo diagnozowani w kierunku współistnienia innych
endokrynopatii, a szczególnie raka rdzeniastego i pierwotnej nadczynności przytarczyc - składowych zespołu MEN 2A.Introduction: Pheochromocytoma is rare tumor with a highly variable clinical presentation. This report provides clinical picture, efficiency
of diagnostics and treatment of pheochromocytoma in 8-years in the endocrinological center in Wroclaw.
Material and methods: The records of 37 patients with pheochromocytoma were identified, who were treated in 2000-2007 in the Department
of Endocrinology, Diabetology and Isotope Treatment in Wroclaw. There were 23 women (age 23-75 year) and 14 men (age 17-74).
We studied frequency of clinical signs, usefulness of diagnostic methods and efficacy of treatment.
Results: The duration of the clinical history ranged from 2 months to 16 years. The most frequent symptoms were: hypertension paroxysmal
and constant, palpitations, headache, sweating and anxiety. The most sensitive diagnostic method was increased concentration of
urinary metanephrine in 24-hour urine. Computed tomography was the most widely used method for tumor localization. Adrenal pheochromocytoma
was detecting by CT in all patients, predominated in right adrenal, in 1 case in urinary bladder. Surgery caused remission
of hypertension in 59%, improvement in 26.8%, and no changes in 13.9% of patients. Malignancy was reported in 2 cases, 1 woman died
after surgery. MEN 2A occur in 21.6%.
Conclusions: The diagnosis of pheochromocytma is usually made after long duration of the disease. The study confirms that clinical
presentation of pheochromocytoma is variable and nonspecific, this finding makes the diagnosis very difficult. The most typical symptom
is paroxysmal hypertension, which is present only in 40%, other symptoms are nonspecific. The measurement of 24-hour urinary metanephrines was the best indicator. CT was almost always successful in localizing the tumor. Patients with pheochromocytoma should be
consider for other endocrine diseases especially medullary carcinoma, primary hyperparathyroidism and other component of MEN 2A
Erythema elevatum et diutinum in a young man coexisting with autoimmune polyglandular syndrome type 3 – case report
Introduction. Erythema elevatum et diutinum (EED) is classified as a variant of leukocytoclastic vasculitis. The etiology of this disease is unknown. Erythema elevatum et diutinum may coexist with several systemic disorders including hematologic and rheumatologic diseases as well as type 1 diabetes, thyroid diseases and other endocrinopathies. Autoimmune polyglandular syndromes (APS) are rarely diagnosed conditions characterized by the coexistence of at least two autoimmune endocrinopathies and non-endocrine autoimmunopathies.
Objective. Presentation of a patient with EED coexisting with APS type 3.
Case report. A 23-year-old male patient was admitted to our department due to nodular lesions lasting for 5 months, located on the extremities, which were diagnosed clinically and confirmed histopathologically as EED. In spite of skin lesions the patient suffered from diabetes mellitus type 1, hyperthyroidism, celiac disease, myopathy and idiopathic urticaria – abnormalities characteristic for APS type 3. Substantial clinical improvement was observed after systemic administration of dapsone and, due to upper respiratory tract infection, a few weeks of antibiotic therapy.
Conclusions . We present this case due to the rarity of EED, especially coexisting with APS, and the good effect of therapy with dapsone and oral antibiotics
Pyoderma gangrenosum, diagnostic and therapeutic difficulties – two cases imitating breast cancer
Introduction . Pyoderma gangrenosum is a rare dermatosis with unknown etiology. Painful and quickly spreading ulcerations with necrotic bottom and heaped wound edges are typical.
Objective . The aim of this study is to present patients with pyoderma gangrenosum which imitated breast cancer and their treatment.
Case report . Two women suspected of having breast cancer developed painful and non-healing ulceration after surgical excision of nodules. Although in histopathology mastopathic changes were described, the diagnosis was verified and subsequent biopsies showed no cancerous changes. Ulcerations were treated as an infectious surgery complication. The diagnosis of pyoderma gangrenosum was made on the basis of characteristic anamnesis, clinical features and histopathological examination. In the treatment cyclosporine with dapsone and cyclosporine with glucocorticosteroids were used and a very quick local response and pain relief were obtained.
Conclusions . The presented cases illustrate diagnostic difficulties and the role of the pathergy phenomenon in the development of pyoderma gangrenosum lesions. The course and treatment of the disease in both cases also reveal the ineffectiveness of monotherapy with glucocorticosteroids
Chronic graft-versus-host disease – the role of dermatological treatment
Introduction . The number of allogeneic haematopoietic stem cell transplantations is increasing year by year. The most common long-term complication is chronic graft-versus-host disease, with skin involvement noted in over 90% of cases.
Objective . To present and discuss skin lesions occurring in chronic graft-versus-host disease with a particular focus on available dermatological treatment modalities.
Case report . A 45-year-old male patient, who received allogeneic haematopoietic stem cell transplantations in December 2012, presented for the treatment of skin lesions secondary to chronic graft-versus-host disease. Clinical examination revealed hyperpigmented lichenoid eruption on the trunk; irregular patches of non-scarring alopecia; poikiloderma on the face and upper limbs; and sclerodermatous lesions on the hands with diffuse epidermal exfoliation on the palms and soles. Histopathological examination confirmed the diagnosis of chronic graft-versus-host disease. Treatment consisting of phototherapy and intensive topical treatment was introduced.
Conclusions . Skin lesions secondary to chronic graft-versus-host disease may impair the daily functioning of post-transplant patients. Comprehensive therapeutic management should be based on the cooperation between haematologists and dermatologists
